New Patient Registration
We welcome new patients and encourage you to register. We cover a wide geographical area including Lower Horsebridge, North Hailsham*, Hellingly, Rushlake Green, Dallington, Windmill Hill, Boreham Street, Wartling and all the villages in between, including of course, Herstmonceux.
*For patients in North Hailsham, please speak to our reception team to establish if you are within our boundary.
To register at Herstmonceux Integrative Health Centre, a registration form and patient questionnaire form will need to be completed . You will need to provide photographic proof of identity and a current utility bill, all adults will need to register in person. All the relevant forms can be collected from our Health Centre.
The practice operates a policy of no discrimination regardless of race, gender, social class, age, religion, sexual orientation, appearance, disability or mental condition.
When you register you may be required to have a ‘New Patient Check' appointment with the practice nurse, which is an informal consultation to gather information regarding your previous medical history, regular medication, to check your blood pressure and urine and to find out how best the practice can serve your medical needs.
All patients over the age of 75 are encouraged to attend the surgery on an annual basis at which time a medication review and general check-up will be performed.
