We welcome new patients and encourage you to register

Our practice vision is a world where people have the tools to heal and maintain their wellbeing,  where communities are connected, self-supporting and people are empowered to live sustainably for the benefit of all. While you are registered with us we will do everything we can to support you in taking care of your health for now and the future.

How to register with us 

  • Complete a GMS1 Family Doctor registration form for each family member
  • Complete a patient questionnaire form for each family member
  • All adults (over 16yrs) are required to bring completed forms to us, in person, along with photographic proof of identity and a current, last 3months, utility bill.  

If registering a child under 16yrs please bring their red book or birth certificate.  Please allow one week for registrations to be completed, and ensure you have enough medication, if required, from your current GP.
